You wish to submit an application to the Social Insurance Institution (ZUS)? Or maybe you wish to check whether your employer has registered you for insurance? Remember that much of this can be done online. All you need to do is create an account on the Electronic Services Platform (PUE) and confirm your identity.

The portal is targeted at

  • insured persons (for example, workers, contractors),

  • contribution payers (for example, employers, persons carrying out non-agricultural activities),

  • recipients of benefits (for example, old-age pensioners and disability pensioners),

  • doctors,

  • bailiffs.

A contribution payer is any company, institution or person that registers insured persons with the Social Insurance Institution and settles and pays social or health insurance contributions for them. It may be, for example, an employer or a person carrying out non-agricultural activities.

What you will find on the portal

The PUE allows you to settle many matters relating to social and health insurance. Your profile will feature a tab suitable for each role (insured person, recipient of benefits, contribution payer, doctor, bailiff). The tab will contain data and services that you may use.

As the user of a PUE ZUS profile, you may view your roles and powers of attorney (commission of plenipotentiary) granted. You may also grant a new authorisation to a person you designate. All you need to do is complete a ZUS PEL power of attorney form and send it to us.

PUE ZUS PORTAL FOR INSURED PERSONS (for example, workers, contractors)

The ‘insured person’ tab allows you to:

  • check the status of your account with the ZUS;

  • check information on insurance for which you have been registered by the employer or contracting authority and check information on the social and health insurance assessment basis and contributions;

  • check whether your family members have been registered for health insurance;

  • check information on sick notes and sign up for an SMS or email subscription to receive messages when a doctor issues an electronic sick note (e-ZLA);

  • calculate your projected old-age pension using an old-age pension calculator;

  • submit an application for the Family 500+ programme,

  • create a confirmation of your data in the ZUS system by yourself and send it to an institution that requires such a confirmation.

PUE ZUS PORTAL FOR RECIPIENTS OF BENEFITS (for example, old-age pensioners and disability pensioners)

The ‘recipient of benefits’ tab allows you to:

  • check information on the benefits granted and paid to you (old-age pensions, disability pensions, benefits);

  • access PIT forms;

  • check whether your family members have been registered for health insurance;

  • submit an application for a change of data required for the payment of benefits, for example give your bank account number.

PUE ZUS PORTAL FOR CONTRIBUTION PAYERS (for example, employers, persons running businesses)

The ‘payer’ tab allows you to:

  • check the current settlement status;

  • check information on persons registered for insurance; access your workers’ electronic sick notes (e-ZLA);

  • submit an application for a social security clearance certificate;

  • complete and send insurance documents via the free ePłatnik application (if you register up to 100persons for insurance);

  • create a confirmation of your data in the ZUS system by yourself and send it to an institution that requires such a confirmation.

PUE ZUS PORTAL FOR DOCTORS

The ‘doctor’ tab allows you to:

  • issue electronic sick notes and certain applications, for example applications for therapeutic rehabilitation as part of the ZUS’s disability prevention measure;

  • download a free certificate from the ZUS system to sign e-ZLA sick notes and, for example, e-prescriptions.

This tab can also be used by medical assistants authorised by doctors to issue sick notes on their behalf.

PUE ZUS PORTAL FOR BAILIFFS

The ‘bailiff’ tab allows you to:

  • obtain the necessary data for enforcement cases you are handling;

  • submit a request for information on insured persons or payers against whom you are conducting enforcement proceedings.

  • In order to use the PUE platform, create a profile on the PUE ZUS portal. You must be over 18 to register and confirm a PUE ZUS profile.

How to confirm your identity when creating an account

In order to create a profile on the PUE ZUS portal, go to the ZUS website and select ‘Register’.

You may register in the following ways:

  • use a qualified electronic signature;

  • use a trusted profile – login.gov.pl;

  • use electronic banking services (See the list of banks that have concluded agreements with the ZUS);

  • complete a registration form on the website and then confirm the profile within 7days at a ZUS branch – if you do not have a trusted profile, a qualified certificate or an account with a bank that has concluded an agreement with the ZUS. Take your identity card or passport with you to the ZUS branch.

Please remember that you must confirm your identity due to the protection of your personal data. You must have an electronic signature to sign ZUS documents and receive replies.

You may use a trusted profile, a qualified certificate or a personal signature (e-identity card).
